چکیده

Massive bleeding is a serious medical complication arising from trauma, surgery, and invasive procedures. In this case, timely effective hemostasis must be applied to patients. The aim of study compare transfusion therapies guided by thromboelastogram (TEG) conventional coagulation tests (CCTs) during orthopedic focusing on blood-component function patients who underwent surgery in our hospital May 2019 November 2020 were retrospectively analyzed. According different transfusion-guiding strategies, the assigned into CCT group containing 214 TEG 266 used fewer volumes blood products including red cell (RBC) suspension, fresh-frozen plasma, cryoprecipitate, apheresis platelets than ( P < 0.05 ). After hemoglobin (Hb) RBC counts decreased, but white (WBC) increased all receiving whether therapy or CCTs. Importantly, exhibited WBC concomitant with higher Hb more id="M2"> There was no significant difference platelet (PLT) between two groups before after id="M3"> > Significant declines thrombin time (TT), partial activated thromboplastin (PATT), prothrombin (PT), d-dimer (D-D), along an increase fibrinogen (FIB) observed surgery. showed reduced TT, PATT, PT, D-D, elevated FIB compared id="M4"> Posttransfusion, id="M5"> K value (time reach certain clot strength) id="M6"> R (coagulation reaction time) angle (α) (clot formation rate), MA (maximum amplitude), CI index) id="M7"> When liver assessed, it found that levels alanine aminotransferase (ALT), aspartate (AST), total serum bilirubin (TBIL) significantly, albumin (ALB) decreased lower ALT, AST, TBIL level ALB id="M8"> With regard renal function, had creatinine (Scr) urea nitrogen (BUN) declined uric acid (UA) surgery; however, Scr BUN UA id="M9"> view above data, TEG-guided could reduce use products, optimize components, improve for undergoing may prevent dysfunction

Blood transfusion during cardiac surgery: massive transfusion coagulopathy

Methods Total of 406 patients (f/m N=103/303) were studied in the ICU in 2011; 21 (5.17%) males were observed,. HGB in OR prior to surgery was: 127.43 g/L(SD±19.4; range 85-165; median 129g/L). Patients were divided into two groups, according to the quantity of RBCs received. Group 1 (N=15) received 5-9 units RBCs; group 2 (N=6) received ≥10 units RBCs. EuroSCORE II was used for operation risk ...

Do antifibrinolytics reduce allogeneic blood transfusion in orthopedic surgery?

Studies have shown that antifibrinolytic (aprotinin, tranexamic acid, epsilon-aminocaproic acid) reduce blood loss in orthopedic surgery. However, most lacked sufficient power to evaluate the efficacy and safety on clinical outcomes. This meta-analysis aims to evaluate whether intravenous antifibrinolytics, when compared with placebo, reduce perioperative allogeneic erythrocyte transfusion requ...

Thromboelastography-guided transfusion algorithm reduces transfusions in complex cardiac surgery.

UNLABELLED Transfusion therapy after cardiac surgery is empirically guided, partly due to a lack of specific point-of-care hemostasis monitors. In a randomized, blinded, prospective trial, we studied cardiac surgical patients at moderate to high risk of transfusion. Patients were randomly assigned to either a thromboelastography (TEG)-guided transfusion algorithm (n = 53) or routine transfusion...

[Factor XIII-guided treatment algorithm reduces blood transfusion in burn surgery].

BACKGROUND AND OBJECTIVES Major burn surgery causes large hemorrhage and coagulation dysfunction. Treatment algorithms guided by ROTEM® and factor VIIa reduce the need for blood products, but there is no evidence regarding factor XIII. Factor XIII deficiency changes clot stability and decreases wound healing. This study evaluates the efficacy and safety of factor XIII correction and its repercu...

Massive blood transfusion therapy.

Surgical patients requiring massive blood transfusion therapy present many challenges for the anesthetist. The decision to transfuse homologous banked blood and its components must be weighed against the potential complications that may occur in this form of therapy. A review of metabolic changes that occur in banked blood, the risk of infection, and physiologic derangements that may develop du...
